Ghanaian youth midfielder Thomas Partey has reached the final of the 2015/16 Uefa Champions League final after he came off the bench to help Atletico Madrid snatch the berth from Bayern Munich at the Allianz Arena on Tuesday night.

The 22-year-old replaced French international Antoine Griezmann 21 minutes from time as Atleti snatched a dramatic triumph.

Diego Simeone's men sealed their place in the Champions League final despite suffering defeat at the hands of Bayern Munich after Antoine Griezmann struck in the second half.

Xabi Alonso put the German side ahead in the first half but Griezmann levelled things up in the second before Robert Lewandowski put the home side back ahead.

Atletico could have put the game to bed but Fernando Torres missed from the spot.

Partey has become the sixth Ghanaian player to reach the Uefa Champions League final after Abedi Pele (Marseille), Ibrahim Tanko (Dortmund), Samuel Osei Kuffour (Bayern Munich) Michael Essien, (Chelsea) and Sulley Muntari (Inter Milan).

Partey has played influential role in Atleti's campaign, starring in cameo appearances for Diego Simeone's side.
